We just moved here and I am so impressed with the overall care. I worked for a health system for 30 years and can honestly say that our healthcare system was lacking in patient satisfaction and just could not get it right. The care I received was exceptional. I thank Dr. Mustain and the staff.

After leaving my job at the end of 2025, I needed to find a primary care physician and facility to take care of me after being with the same care team for the past ten years. Everyone within the Gunderson Health System/Emplify has been awesome. They're friendly, knowledgeable and helpful. Dr. Mustain was extremely easy to talk to and very attentive to everything I said. I hope to not have to see him more than for my annual wellness exam, but I'm confident that whatever I may need him for, he'll be an advocate for what is the correct care for me.

I would like to be able to turn off the emails I get to check in. I do not have my email on my phone, so I do not use this and find it annoying! What is the point if I have to check in at the desk anyway when I get there?!

The only thing I had a issue with is that the provider did not look in my child's ears. She has been fussy for the past 4 days, shes getting teeth but but very fussy. Now I either make another appointment or go to urgent care.

This was my first visit as I transferred from Mayo system. Pleasantly surprised and liked Dr Mustain

The follow-up message seemed like a flippant response from a nurse rather than having been actually being shown to the doctor for a person response.

Dr Mustain was very aggressive in reviewing my medications and changed several dosages and even eliminated one medication altogether. I am pleased about these changes provided they work out for me.

That was probably one of the best doctors visit I've had. I really appreciated that doctor and I am considering making him my primary care physician.

The Holmen Clinic is one of the best clinics I've ever been to. Everyone is kind, caring, and professional and my provider is absolutely amazing.

Dr Mustain was great, explained everything to me and was on time and down to earth. Not a lofty academic. Seemed to me he knows medicine.

I didn't use any of the digital features

Dr. Mustain was my doctor for many years at the SSM clinic in Baraboo ... I specifically came to the Holmen clinic to continue the EXCEPTIONAL care he has always provided ! Also, kudos to Pam at reception !! I hadn't been to your clinic before and got lost on the last leg of my trip. Pam stayed on the phone with me for 15+ minutes helping me navigate my way until I pulled into the parking lot.

I did not like having to go to a different clinic and a completely new physician that was very unfamiliar with my history. I understand your physicians are very busy but now I'm unsure if I am getting unnecessary additional tests and some that are an additional expense to me.

Dr. Mustain gave me more time than I expected and was exceptional is his attention to detail and caring demeanor.
