This was the first time I had a visit with a provider about my health issue where I left feeling like I was doing things right and on a good track.

Very hard to hear over the video call what was being said.

Provider was excellent! He is very good at listening to the patient. Provided great explanations.

Dr. Haque a is down to earth person who talks on yr level as a patient and has a sense of humor. ?

He provides a lot of information about my condition.

I have had many appts. With my Dr. He is one of the best drs. I have ever had. Very thoughtful, kind and informative. I completely trust him and his knowledge. I would like all my health care staff to be like him. Thank you and God bless the whole team!

I feel like I have a team of people behind me helping me to get/stay healthy. I was even offered a flu shot, which saved me time and effort.

The e-check-in system now asks for specific travel during the last thirty days: places and dates. When I get to the desk, I am asked again, even though the information is in front of the receptionist. Seems redundant, especially with no explanation as why this info is needed.

I was asked to make lab appts

I have asked various people for maps to help me learn my way around a new clinic for me and they all say you don't have maps to hand out. Please create and provide maps for new patients to learn their way around the clinic, as it is so easy to get lost and mixed up with the various hallways and elevators that aren't linked. Thank you.

I was extremely disappointed with my visit, I feel that the provider I had has very little understanding of PCOS. I thought that being an endocrinologist he could give me some perspectives on how to tie the PCOS and insulin resistance issues. Before my appointment I could hear him and the student that was with him outside of the door discussing PCOS and that PCOS patients make up Insulin Resistance. Along with comments regarding hopefully I'm not one of these patients that is blaming the PCOS. I called him out on this during our visit because I thought this was quite unprofessional. I feel that in our conversations he did not fully understand how the metabolic functions of PCOS affect many parts of my day to day life and that it is not easy for me to be here trying to get new perspectives to help my symptoms. He really provided me with nothing I don't already know and even stated that "I probably know what he's going to tell me if not more". He also stated that my PCOS is not the cause of my weight gain and when I told him the reason I came is because with PCOS it is very hard for me to lose weight and maintain that. He told me that my PCOS is a symptom of my weight. I am not sure if this is going to be every endocrinologist perspective but I gained nothing from my visit with him and find it difficult to go forward on these issues when I receive this kind of behavior.

Dr. Hacque was wonderful. Was very understanding and answered all my questions. Best appt. I have had in Emplify so far.

I really like Dr Haque. He explains things very throughly so you understand.

The staff and The Doctor was a amazing

Dr Haque is not open to learning or expanding his knowledge to help his patients. He refuses to think outside the box for treatment and focus on non essential issues and not the problem at hand. I don't feel listened to or heard by this provider.

I had a gentleman check me in and he was not very talkative and he never waved me, which I thought was unusual. He just acted totally disinterested. Thank you.
