Dr. Collins was very well acquainted with my eyes and how well they were functioning. He was quite good with expressing what were options for future services, including that my eyes were in fine condition with no need for any short term or even long term treatment.

Dr. Collins did a tremendous job assessing my eyes to determine the cause of my blurriness. I appreciate the extra time he took and explanation to me given my lack of knowledge regarding the eyes!

Had to wait a while after check in to be called.

I saw chipped paint, dust, and fingerprints on some of the equipment in the room, and a chipped wood cabinet. It shows a lack of attention to detail, cleanliness and maintenance which lessens the confidence I had in Gundersen, now Emplify. There still seems to be a post-Covid pall lingering.

Everyone there is really nice and wonderful. BUT... I had to wait a really long time to see the provider. I was there at 7:30 AM and first had to do some testing - which took about 20-30 minutes. I didn't see the provider until around 9:20. I had another appointment in Onalaska at 10:00 and I seriously thought I was going to have to leave before seeing the provider. I'm not sure exactly what time I was on his schedule because MyChart only showed that my appt was at 7:30. Luckily, I did manage to get out on time, but it seems crazy that an eye appt should take over 2 hours. I'm sure at least an hour of that time was spent waiting. However, I do want to reiterate that every person there was very nice and I have no plans to switch my care elsewhere. :-)

Both the technicians and Dr. Collins were very thorough, very friendly, and they didn't seem like they were hurried, were both very down to earth and both did an excellent job.

I am extremely thankful for ability to be seen same day in Optometry for new change in my vision. Dr Collins was very helpful in examining my eye, having retinal photos taken and then explaining to me what happened in the back of my eye to cause vision change. Thank you for rapid access to Optometry Clinic as well as Dr Collins' helpful evaluation and explanation.

Dr. Brad Collins is very thorough and professional! He is thoughtful and considerate and I look forward to my annual wellness check with him checking my eyesight.

Excellent staff. Website is clunky, repetitive, invasive, gives false information, is user unfriendly, and VERY POOR CUSTOMER SERVICE.

I was a walk in due to an eye problem and was seen with 20 minutes of check in. Dr Collins was great.

I like Dr. Collins-he has a good sense of humor that helps move things along. He explains things well-I know my cataracts are getting worse but not yet ready for surgery. That was my main question.

Dr. Collins was an aggressive, take care of me type of doctor. He realized that, I'm sorry, I realized that maybe my old ophthalmologist was just coasting to a retirement. Dr. Collins was aggressive in treating me, letting me know what I got to do, what could happen, and what's going to happen. And then he went out of his way to get one of his colleagues to give me a laser treatment. And after 24 hours from that, I'm feeling and looking and reading a lot better because of him. I hope he stays at Gundersen for a long time because this man deserves to be in that department and I'm glad he's my new ophthalmologist. Thank you.

Dr. Collin's male technician is in the wrong profession.

Dr. Collins has been a huge help to me as I've aged and encountered new vision issues. He encouraged me to have two cataracts removed by the best doctor to do this work; then recently had me see an excellent doctor to give me lazer treatments in both of those eyes. Dr. Collins has been my "go to" vision doctor for more than a decade, and has always been highly professional, thorough and very competent. I have recommended Dr. Collins to a half dozen or more of my friends and colleagues, and will continue to do so as he is a terrific eye doctor in every way.

Dr. Collins is very good. The tech lady that did the initial eye checks is also very good at what she does. Both know their trade and have great, caring personalities. I'm sorry I can't remember her name.

My experience at Emplify is always pleasant. From the front desk to the nurses then the Drs. Everyone is more than helpful & pleasant!!!

It always amazes me how the technology changes between my annual eye appointments. So reassuring to know I am being examined using new and/or improved procedures. From start to finish my appointment was very smooth and I was treated as a valuable person.

Everything was excellent, just wonderful.

Dr Collins is very good ?

I still find parts of MyChart to be cumbersome and/or confusing to find things. It's good in many ways, but finding some post visit information isn't the best.

I wish you would add additional vision insurance. You used to accept VSP, then I thought BCBS vision but you didn't. I would prefer to have my glasses prescription filled by you but you don't take any other insurance!!

Our visit with Dr. Collins, as usual was exceptional. He knows my Mom's history very well and does his very best to keep her eyes comfortable. He treats her kindly and with respect. Also, his upbeat attitude is great!

Dr. Collins was very, very conscientious and he didn't just rely on the measurements from the machines or the meters. He actually looked and saw how those measurements, how they really related to me and made some adjustments. I was very, very happy to be with Dr. Collins. Thank you.

Visit went smoothly, Dr answered my questions in a meaningful way and I was very satisfied with the whole proceedure all the staff.

I was not happy that I wasn't notified that it was 3 years since I had an eye exam. I never received a notification I was due for one. Evidently there was a glitch in the system. I would hope that the welfare of your patients would have been considered when these occurred. Maybe checking records to see if everyone was updated?
